Authorization Details

By completing this form, you authorize Upstate Energy to charge your credit card for the agreed-upon amounts for deliveries, labor, installations, repairs, or deposits. You also acknowledge that this authorization remains in effect until you provide written notice to Upstate Energy of its termination.
